Making a referral

When filling out our referral form information, we ask you attach previous evaluations completed of the patient. For example, this may include speech therapy evaluations, occupational therapy evaluations, psychological or neurological evaluations, IEP evaluations, and any important medical information regarding the patient's development. Gathering historical information of the patient's development is imperative in providing a thorough, ethical evaluation. If a community provider is making a referral, please attach an ROI so we can connect with you. Evaluations will take place in Bloomington at our office location, unless scheduled otherwise.
